Setup: Nursing Assessment and Service Plan

Learn how to setup the Nursing Assessment and Service Plan.

Nursing Assessment Setup

  1. Click on "Community Record" under the "Community" tab.
  2. Click on “Setup: Assessments/Plans” under “Care” in the “Setup” dropdown.
  3. Fill out the form.
    • The "State assessment" section will soon be functioning, but for now keep "Custom" selected.
    • The "Select" section contains a list of items that can be selected to display on the Nursing Assessment.
      • Among the options are types that deal with basic body systems and ADLs. It is important to remember the differences between these types when creating a Service Plan later.
      • An asterisk(*) signifies identical options for the same section. Some states use different names for different sections, hence the need to display them. Simply choose one or the other.
    • The section below contains a text box for each selection made in the "Select" section. The text in the text box will be displayed if there are no issues with that particular section on the Nursing Assessment.
    • The Nursing Assessment needs to be reviewed every so often. This interval can be set in number of days, and a notification will appear once the amount of days has elapsed.
    • The "Copy Forward Nursing Assessment" function allows the first Nursing Assessment, once saved, to be copied forward to a continuing Nursing Assessment. Some states allow this, and some don't.

Service Plan Setup

  • 4. Continue to fill out the form.

    • The "Select" section is associated with ADLs. Any ADL types selected in the above "Select" section should correspond with the selections made in this "Select" section.
    • The "Show Times" button in the "Time Individual ADLs" section can be selected. This will show the time that an ADL was given.
    • The Service Plan needs to be reviewed every so often. This interval can be set in number of days, and a notification will appear once the amount of days has elapsed.
    • The "Configure Service Item Times" contains a list of times that will control when an ADL can be scheduled.
    • The button in the "Hide Occurrences" section controls whether or not the Occurrences are shown on the Service Plan.
    • Diagnoses relating to ADLs will display on the Service Plan if the "Show Diagnoses…" button is selected.
  • 5. Click the "Save" button to finish.
