Creating a New Service Plan

Learn how to create a new service plan. The service plan needs to be set up in the Community Record before a new service plan can be created. See the Setup: Nursing Assessment and Service Plan help topic if not set up yet. It is also ideal to have completed a nursing assessment before creating a service plan, however it is possible to create a service plan independent of a nursing assessment.

Navigate to the Service Plans

  1. Click "Select Resident" under the "Resident" tab.
  2. Select a resident.
  3. Click "Service Plan" under the "Nursing" dropdown.
  • All service plans for this resident are displayed here.

Edit an Existing Service Plan

  • Existing service plans that are incomplete do not have a completed signature in the "Service Plan Complete" column.
  1. Click on the pencil icon to complete and finish existing service plans.
  2. Fill out the form.
    • By default the sections are dormant. To edit a dormant section, simply select "Active." Sections that are already activated have been addressed in the corresponding nursing assessment. Select "Inactive" to inactivate a section.
    • In some sections information is already filled out from the nursing assessment. Information can be added, and existing information can be edited.
    • Usually most of the sections are filled out with information from the nursing assessment. Scheduling the ADLs is usually the only thing left to do on an existing service plan. To schedule the ADLs, select the desired time(s) the ADLs should be done.
    • Unique service items can be added to individual residents.
      1. Click on the "Add Service Item" button.
      2. A blank service item will be added at the bottom of the form.
      3. Enter information for the new service item.
    • If required by the state, diagnoses can be added to each ADL.
      1. Click on the "Add New Diagnosis" button.
      2. Fill out the form.
      3. Click the "Save" button to save the diagnosis and return to the service plan.
      4. In any section on the service plan, click the "Update Diagnosis" button and select the diagnosis that corresponds with the ADL.
    • Click the "Save" button to save progress made on the service plan.
  3. Select the "Service Plan Complete" button, and click the "Save" button to save the service plan and finish. Once the service plan has been saved like this it will be locked, and ADLs will begin to be generated.

Create a New Service Plan

  1. Click the "New Entry" button.
  2. Fill out the form.
  3. Select the "Service Plan Complete" button, and click the "Save" button to save the service plan and finish.
