The Behavior Plan Tracking System in BlueStep is a comprehensive behavior management tool that allows for the identification and evaluation of behavioral symptoms.
Create a List of Behaviors
Click on “Community Record” under the “Community” tab.
Click on “Setup: Behaviors” under the “Setup” dropdown.
Click on the “New Entry” button to add new behaviors to the list.
Enter a behavior.
The “Active” button can be selected or deselected to activate or deactivate a certain behavior.
Click the “Save” button to finish.
Create a Behavior Plan
In a resident’s Record Summary, click on “Behavior Plan” under “Behavioral” under the “Nursing” dropdown.
Click on the “New Entry” button to create a new behavior plan.
Fill out the form.
When a “Next Review Date” is entered, staff will receive a notification to review the behavior plan. See Setup: Resident Warnings for more information on notifications.
The list of behaviors is displayed in the “Behavior” dropdown.
More behaviors can be added to the list by clicking on the “Add New Behavior Option” button.
Medications specific to a behavior may be selected in the “Intervention Medication” dropdown. This dropdown will display the resident’s active medications.
Click on the “Add Trigger” button to add a behavior trigger. Multiple triggers can be added after clicking the “Staff Signature” button for each trigger.
Click on the “Add Intervention” button to add a behavior intervention. Multiple interventions can be added after clicking the “Staff Signature” button for each intervention.
Click the “Save” button to finish.
Completed Behavior Plans
The completed Behavior Plan will be listed as an active Behavior Plan. Multiple behaviors can be tracked for each resident.
Once a Behavior Plan has been signed and saved, new triggers or interventions can still be added. Old triggers or interventions can be archived by entering an end date.
The “Nursing Review” allows for a new review date to be entered.
The review notes can be copied to the nursing notes report by selecting the “Copy Review Note to a New Nursing Note” button.
The new Behavior Plans can be accessed from the “CareDash” page in the “Resident Alerts” module.
The Behavior Plan warning is triggered when a behavior review is coming due or past due. See Setup: Resident Warnings for more information on notifications.
The Occurrences warning is triggered when a behavior is documented. See Setup: Resident Warnings for more information on notifications.
Charting Behaviors
On the “Charting” page, click on the Behaviors” tab.
Every resident with an active Behavior Plan appears here. Each resident’s Behavior Plans display under their photo.
To chart a resident’s behavior occurrence, click on the corresponding behavior.
Fill out the form.
Select the trigger.
Select the intervention and whether effective or not.
Click the “Staff Signature” button.
Click the “Save” button to finish.
Discontinue a Behavior Plan
Enter the Behavior Plan that will be discontinued.
At the bottom in the “Discontinue” section, click on the “Nurse Signature” button.
Enter any notes in the DC Note memo box.
Click on the “Copy DC note to a new Nursing Note” if desired.
The “Copy DC note to a new Nursing Note” and “Add New Nursing Note” buttons allow you to create or copy a Nursing Note from the Behavior Plan. Previous Nursing Notes pertaining to this Behavior Plan can be viewed at the bottom of the plan.
Click the “Save” button to finish.
Behavior Reports
A report with all of the behavior occurrences of all residents can be found under “Resident Reports” under the “Reports” tab.
Click on the “All Behavior Occurrences” link.
Click on the “Search Settings” dropdown to refine your search by date, behavior, effectiveness, etc.
